Worker's Compensation
This section provides additional instructions and answers to frequently asked questions about employee injury reporting and the workers' compensation claim process.
Student workers, graduate assistants/associates, and part-time faculty and staff who are active with University of Arizona payroll can file a workers' compensation claim if an injury occurred during the scope of their employment. Claim compensability will be determined by the State of Arizona Administration, Risk Management Department.
Supervisors and other department administrators are authorized to submit online injury reports. [See Quick Reference Guide]
- Tell your supervisor about your injury as soon as possible.
- Get medical treatment if needed, see below for treatment options.
- Ask your supervisor to submit an online injury report.
- For emergencies, call 9-1-1 for paramedic response and ambulance transportation to the nearest emergency room or trauma center.

- Departments with operations outside the Tucson area are encouraged to plan ahead of an injury and identify appropriate injury treatment options in their immediate area. Treatment may be obtained at any urgent care center, occupational health clinic, hospital, or other healthcare provider.
- Tell the medical care provider that your injury is work related.
- Provide the treating physician with information about how the injury happened.
- The employee will fill out the top portion of a form called “Worker’s & Physician’s report of injury for the Industrial Commission of Arizona (102)” and the physician will complete the bottom portion and mail it out to the appropriate agencies.
The workers' compensation treating physician may give you a referral for physical therapy, radiology, other diagnostic exams, or to see a specialist. Please fax this referral to 602-382-2370. If you do not have access to a fax machine, you may send the referral to Anna Llamas at workcomp@list.arizona.edu. The State Risk Management utilization review will determine approval and contact the provider with authorization. This process generally takes 3-5 business days. If you do not hear from the provider regarding the appointment, please contact State Risk Management at 602-542-2182.
The State of Arizona, through the Arizona Department of Administration, Risk Management Division (ADOA-RMD) manages workers' compensation coverage for all state employees. ADOA-RMD is also referred to as State Risk Management.

Upon receipt of the supervisor report of injury/illness, UA RMS will forward to State Risk Manangement and a claim will be created for those injured employees who have received or are planning on seeking medical care. Notification of the claim's status (accepted or denied) is mailed to the injured employee's home address from State Risk Management. If the claim is accepted for benefits, the notification will provide the claim number and the name and contact info for the assigned claims representative. The claims representative monitors the medical status of the injured employee, and must authorize all medical treatment under the claim.
The name and contact information of the claims representative at State Risk Management is included with the notification of claim acceptance.

- Tell your supervisor you have been taken off work.
- Provide a copy of the off-work slip from the doctor to your supervisor and to RMS.
- Ask the doctor if you are able to perform modified or restricted duty.
- Following prescribed treatment, medications, physical therapy to get well and back on the job as soon as possible.
- Avoid activities that might worsen your injury or slow your healing time. Examples might include work around the house, recreational activities, hobbies, sports, etc.
- If you have a second job, or other source of income, this must be disclosed to your claim representative.
- You may not leave Arizona for a period exceeding two weeks "while the necessity of having medical treatment continues, without the written approval of the Industrial Commission of Arizona."
Your treating physician must authorize your return to work and provide a written return to work status report. A copy must be provided to your supervisor, and to RMS. Your return to work may be with modified, restricted, or full duties, as directed by your physician.
If an employee is off work for seven or fewer calendar days for a work-related injury, workers' compensation benefits for lost time are not applicable. Work absence must be reported in accordance with U of A policies for time reporting. The employee may use available sick/vacation leave for the first seven days.
After a claim is accepted for benefits and the treating physician has placed the employee off work workers' compensation becomes payable when the disability lasts beyond seven days. If the employee is off work between 8 - 13 days, the employee is compensated for the days over seven. If the disability extends beyond 14 days, compensation is retroactive to day one. The employee is entitled to 66 and 2/3rds of their average monthly wage. By statute, the Industrial Commission reviews and established the maximum average monthly wage on a yearly basis.
Industrial leave pay is processed through the employee's regular paycheck through the normal University's pay cycle. The workers' compensation analyst will calculate the 66 and 2/3rds workers' compensation to industrial leave hours (ILP) and enter the ILP hours into the employee's time record applicable pay period. The employee will use accrued sick leave then vacation leave balances, if available, for the remaining 1/3rd of time to make up the difference to equal their regular pay. If the employee has no accrued sick and vacation leave available, the 1/3rd time is unpaid. During unpaid time, the employee is not eligible for sick /vacation time accruals. If the employee is receiving workers' compensation during a holiday or University closure, the employee will receive the industrial leave pay instead of the holiday or closure pay. Industrial leave pay is non-taxed. Regular employee deductions will continue without disruption during this pay status.
