Report an Injury, Incident or claim to Risk Management  for the following:

• Employee Incident/Injury Reports: report on-the-job injuries or incidents involving UA employees.
• Property Loss Notice: report loss, damage, theft, or vandalism to UA facilities, buildings, or business personal property
• Auto Loss Notice: report any loss or physical damage to university-owned vehicles.  
• Auto Loss Notice - Glass Only: report claims explicitly related to damaged auto glass (windshields, rear glass, side windows).
• Non-Employee Incident Report: report injuries or incidents involving non-UA employees, such as students or campus visitors.
