Effective January 4, 2019, Risk Management has implemented a new system for reporting injuries, incidents, and claims for the following:
- Employee Incident/Injury Reports: report on-the-job injuries or incidents involving UA employees.
- Property Loss Notice: report loss, damage, theft, or vandalism to UA facilities, buildings, or business personal property
- Auto Loss Notice: report any loss or physical damage to university-owned vehicles.
- Auto Loss Notice - Glass Only: report claims explicitly related to damaged auto glass (windshields, rear glass, side windows).
- Non-Employee Incident Report: report injuries or incidents involving non-UA employees, such as students or campus visitors.
