Windshield Claims

Our insurance covers windshields and windows on University owned vehicles provided the replacement exceeds $100. Our insurer provides an expedited claims procedure for vehicle glass damage. Departments must follow these steps:

Complete an Auto Loss Report.
Send the completed Auto Loss Report to Risk Management & Safety. You may want to fax the claim to (520) 621-3706.
Call Risk Management & Safety at 621-3482 for the account number to give Safelite for your claim. Risk Management and Safety will only give out the account number with an Auto Loss Report from your Department.
Contact Safelite from the list of either Safelite offices or use Safelite Mobile Services (PDF format) and arrange for the work. The lists for offices and mobile services throughout Arizona include addresses and telephone numbers. When you arrange for the work tell Safelite this is insurance work on a University owned vehicle and give Safelite the account number from Risk Management & Safety.

Departments using the above procedure will not receive a bill. Departments are responsible for any work of less than $100. If the Safelite amount is less than $100, departments must pay Safelite through a check request or Stores Order (Stores charge a mark-up).

The State's contract with Safelite is competitively bid. Departments using any other vendor may still submit an Auto Loss Report. The insurance recovery will be limited to the amount of Safelite's contract price for the same work. Departments are responsible for any difference between actual costs and the contract price.
