General Laboratory Chemical Safety Practices


3. Recordkeeping

Documentation is necessary and appropriate for both the enforcement of the provisions set forth in this Manual and for the development of information regarding the cause and prevention of workplace injury and illness.


3.1) General Documentation Procedures: Adequate documentation must be kept by each responsible party (see Roles and Responsibilities) to demonstrate compliance with all provisions of the Manual.


3.1.1) The Laboratory-Specific Chemical Safety Training Record Form (PDF format) must be used to document the training of each Lab Worker (see INFORMATION AND TRAINING). Records of the general laboratory chemical safety training are kept electronically via the Laboratory Chemical Safety (LCS) Management System.
3.1.2) The Laboratory Chemical Safety Self-Evaluation Checklist must be used annually to document general compliance with the Manual (see Laboratory Chemical Safety Self-Evaluations). Records of the required annual evaluations are kept electronically when supervisors or their appointed designees complete the Laboratory Chemical Safety Self-Evaluation Checklist on the Laboratory Chemical Safety (LCS) Management System.
3.1.3) The PPE Hazard Assessment Certification Form (Word format) must be used to document surveys to assess the need for personal protective equipment (see Section 1.1.4.6).
3.1.4) The Extremely Hazardous Activities - Prior Approval Request Form must be used to document requests to conduct extremely hazardous activities (see Section 1.2). Records of prior approval requests are kept electronically via the Laboratory Chemical Safety (LCS) Management System.
3.1.5) The Chemical Inventory Form (Excel format) can be used as one means of documenting chemical inventories. Other inventory documentation methods consistent with requirements of Chemical Management Best Practices – Chemical Inventory, are also acceptable.
3.1.6) These records shall be kept for the duration of the employment of those affected.


3.2) Exposure Monitoring and Medical Records: RM&S, Campus Occupational Health Services, and others conducting exposure monitoring or providing medical services, shall ensure that records of these activities are kept, transferred, and made available in accordance with 29 CFR 1910.20, Access to Employee Exposure and Medical Records Standard (for further information, call RM&S at 621-1790).


3.3) Accident Reports: Supervisors must take time to carefully analyze all emergency events, work-related injuries or illnesses, or accidents that could have resulted in injury, even the apparently insignificant occurrences, and ensure that accident reports are filed for each event (refer to the University of Arizona's Workers' Compensation guidelines). The results of such analysis and the recommendations for the prevention of similar occurrences shall be distributed to all who might benefit there from.
